In compliance with the American Disabilities Act Section 504, our mission is to provide services for those students who are academically, physically and cognitively challenged. We strive to develop learning strategies tailored to the needs of each individual student.

Registering for Services
- Complete the Student Intake Summary
- Make an appointment with the Disability Access Center by calling (773) 907-4725, preferably six weeks before classes begin, or as soon as otherwise possible
- Bring Proof of disability (doctor's statement, diagnostic test results, etc.) to the appointment
- Students must register for services each semester

Do's and Dont's for Students
- DO'S
- Notify the DAC as early as possible in the semester if you have a documented disability
- Make sure your documentation is current (within the last five years)
- Notify the DAC if feel you may have a learning disability that has never been diagnosed
- Notify your instructor immediately if you feel you are in the incorrect class level
- Seek help if your disability is causing you difficulty at school
- Contact the DAC
- Speak with your instructor
- Contact the Student Success and Leadership Institute
- Make an appointment with the Tutoring Center
- Understand that we will not release information to anyone about your situation if you are 18 or older, including your parents, unless you sign a release of information form each semester
- DON'TS
- Don't wait until midway through the term to notify the DAC of your documented disability
- Don't expect your disability to be an excuse for poor performance midway through the semester if you have not registered your disability with the DAC
